If there are any other details that have changed, including any custody arrangements or court orders pertaining to your child, please advise the reception staff upon your arrival.
Please note that unless the details of a person (inc. parent/guardian/carer or family member) calling our clinic to enquire about a patient/child are noted on our patient information/consent form, no information will be provided to that person including appointment details or billing information. Any person that you wish to have access to this information should be noted on our consent form.
